A major challenge faced by a lot of the patients we see is when they have multiple chronic illnesses, each with their own medicine regime, and it gets a little overwhelming for them to manage it all. One simple but very practical way that our doctors help is to make a clear simple system for them to organize their meds, and follow a daily plan.
We visited one house recently where for the first time the patients Hypertension is being successfully controlled with medicine. We asked the family what they had done differently than before. They replied, "we realized that when we follow your plan our Mum does better, so now we follow your plan!"
We had also trained her daughter, who is the principal carer in the home, how to best look after her mum. She wants to be a doctor when she grows up, so not only are we helping the patient, but we're also investing in a future doctor!
